It turned out to be a temp directory problem. I moved it from
c:\windows\temp to a directory fully controlled by CVS Users and it
worked perfect. I would suggest commenting on this in the docs
somewhere.

Make sure all CVS users can at least list directories above the CVS
repository all the way up to the root of the drive also.
