[cvsnt] Unicode and Linux pam authentication

Provided your nsswitch is setup correctly then PAM will just work
transparently.  There's no particular need to make a CVS specific version. 
I can find a PAM patch from 2001 but it looks a bit dodgy to me... is there
a more recent one?
